Something is going on with my ds's penis (foreskin technically,) and up until this morning I thought he had probably just yanked a bit too hard and the dot of blood that has been on his diapers was just a result of that. (We're talking head of a pin size of blood drop.)

Then this morning, his foreskin is looking inflamed & cracked and there was an increase in the blood showing on the diaper. Not lots, but enough that I know this just isn't normal anymore. The problem is this - I'm terrified of taking ds to the dr. for fear of them retracting him.

At his 1yr check I specifically told the dr. not to touch or retract his penis before she took off his diaper. When she took off the diaper the tip of the glans was showing the way that his foreskin was laying, and she moved the foreskin slightly and said he was starting to retract. I was uncomfortable about it, but it didn't seem to bother him & it was already done... until reading just a bit ago, I didn't think movement like that could be a form of retraction.

Anyways, that was about 5wks ago. The tiny dots showed up about 1.5wks ago - not on a regular basis, and there was no outward sign other than just some redness on the foreskin which has been pretty typical for him to have where his diaper rests against him. We started trying to prevent his grab & pull actions at diaper changes, and that seemed to make a huge difference & there wasn't anymore blood dots on any of his diapers, which made me think I was right & that he'd probably just been too rough with himself...

Anyways, this morning it's obvious something isn't right. I'd been wondering if maybe the edge of the foreskin is caught inside - can that happen? It looks like the foreskin isn't laying the same - before when I wiped the foreskin was always equally long. (I always wiped downwards so no risk of pushing the foreskin unnaturally.)

Anyways, sorry this is so long, I hope I'm giving enough details to get some help quickly. Like I said, I'm really terrified of taking him to the dr. & making things worse, although at this point it looks like that's what we'll have to do... he was fussy within 15mins of nursing and going back to sleep last night, and looking at his penis this morning I think it's hurting him to pee - having raw looking skin probably hurts. :

So can anyone offer suggestions, ideas? I do have nystatin at home, I could put some of that on & see if it makes a difference in the next few hours. I know something needs to be done, but I don't know what and I'm feeling like I've totally failed him on this. :

Honestly, I wouldn't worry about it. And there's no way you've failed him! To me, it sounds like he's got some normal separation irritation going on. He's a little on the young side but still completely normal for separation of the glans and foreskin to be beginning. When that happens, sometimes the tissue connecting the foreskin and glans can release unevenly, i.e. some places are still "stuck" while others are looser and still others are "free." When he pulls on it, a "stuck" place might come unstuck and cause a little irritation and even a little blood, because it's such a heavily vascular area with a very rich blood supply. It's definitely a very sensitive area and can be painful (think of a minor crack in your lip and how much that can hurt) -- but just because it's painful doesn't mean there's anything that needs treating or is abnormal.

I would just keep an eye on it -- I wouldn't put anything on it or anything. Warm baths with a little baking soda might help, and some nakey time -- but it doesn't sound like there's anything wrong from your description.

You can help with the tip looking dry by putting a good quality barrier cream on it. Just like every other body part the foreskin can get really dried out and that can cause irritation.

Do you use cloth diapers? If you do then they may need stripped if you dont then he may be reacting to some chemical in the diaper. Barrier cream will help with that as well.
