My midwife hasn't asked me to buy one. She did give me a list of supplies I'll need, but whatever I don't already have (and I already have quite a lot of it) I'll be hitting up the dollar store for.
My midwife had me purchase on seperately. I beleive it was around $70. I think the company was called In His Hands or something and they were based out on TX, somewhere near the Hill Country. It had a ton of stuff in it.

I also should have commented that mine was customized for my MW too. She had it available for order through Cascade. I have worked w/ other MWs whose kits were only about $35. We did use nearly everything in our kit.
My midwife had a customized kit for $55 from in his hands. We probably have spent another $50 in linens and towels and misc odds and ends to protect our bed and floors.
I also bought a customized kit the first time and I think it was about $50. But I had some other things I had to get as well. Now that I know where to get stuff and that I can put the supplies together cheaper myself (plus I still have some stuff from the first time), that's what I have been doing.
I got a kit because it was easy and about $35, if I remember. My midwife used her code clamp. The chux, peri bottle and the straws were the most helpful. If I ever have another baby, I don't think I would buy another kit though.
I bought the customized kit for my mw and it was around $50. tbh...if I had considered things more carefully I would have never purchased the kit. There were things I could have purchased cheaper elsewhere and items in the kit that I just plain don't want (sitz bath herbs - I have my own herbs for this, disposable post partum pads - I'd rather use cloth ones).
Then of course, there was the list of other supplies that we had to purchase. Still trying to decide if flannel backed tablecloths are going to be too slippery on our hardwood floors.
My MW's customized kit is going to cost us $32.00. We'll probably spend a little more on the other supplies she has recommended; however, we have most of the stuff at home already.
I didn't even know there were such things. I just asked my midwife what I needed to get and she told me. It wasnt much, she provided things like EPO and brings along all the herbs that she might need to use. I just bought some old sheets at the thrift store to birth on, a shower curtain to protect the mattress, two boxes of sterile gauze pads, cotton "seconds" hand towels and a bottle of olive oil. I think all that cost about $20.
