Prenatal, postpartum forms

Ok ladies. Has anyone else had problems figuring out a format for their prenatal worksheets, etc? My preceptor promised I could use her's as a basis, but she sold her practice and the new lady has only sent me some of the forms and may refuse to send the rest. Doesn't mean I want or need them immediately, but I'm the type who likes to be well prepared and set things up well in advance (sometimes years in advance). I'd rather go back and change something than try to figure it out from scratch when I need it. So anyway, where did you get your forms? Did you make them up based on what you felt you needed and just changed them as your needs changed (I'm talking your forms for keeping track of prenatal appts, labor and delivery, etc). Or did you get them through a preceptor, college, or company? I especially would like to hear from the old school midwives that started out due to a need, rather than recent times when all this schooling and formal apprenticing is available. Thanks for any help.

I can send you mine - just pm me your email address. I got mine from several sources and I still find myself 'tweaking' it from time to time when I need to print off more. I either add information or remove stuff I just dont use. There are alot of midwifery books that have sample forms too - so that would be a good starting point also.

I have developed my forms over many years of knowing what I need not only for myself, but in case my client transfers.
I would be happy to mail or fax you a set if you would like - just let me know.
