The docs in my practice are beginning to test everybody for hsv antibodies with their initial blood work. Here in PA we screen everybody for syphilis which is so much rarer than hsv. But honestly, if you look at actual HSV transmission rates, the highest rates are in women who have never had an outbreak before (ie having their initial outbreak at the time of birth). In the case of the op, I think the actual risk of transmitting, even if the baby came in contact with a lesion is very low. However, given the relatively low risk of acvyclovir in 3rd trimester, I think if I were in her shoes, I would probably take it. It is certainly what I would recommend to a client.
