Client satisfaction forms? anyone? Also, my checklists

Hi,
Months ago, i discussed doing checklists for supplies to take to visits, births, etc. I also was planning on doing checklists for each of the visits, labs, and postpartum visits.

I am happy to report that finally, after much procrastination, I am finished. I intend to upload these to my website today with the help of my teens, so I will update with a link later.

anyone else have these? I'd still love to see others, get more ideas, samples, etc.

And last question: I want to give a satisfaction form out to each client after the birth and postpartum. I have one from DONA I used when i was a doula, and the midwife I apprenticed with has one, but I need to develop my own, anyone have one they would be able to share? what questions do you ask? Do you use a 1-10 scale? Open ended? other?

I would love to see what others do. I have one that I used from another doula and have sinced stopped and want to make up my own.

I have created a postpartum client opinion survey. It is a mix of directed questions with 'rate me from 1-5', and room to write comments. It's 3 pages--pg1 is prenatal, pg2 is labor/birth, pg3 is postpartum care. Each area has about 10 'rate me' questions. At the end, I ask a couple of yes/no questions: is my fee fair for what i provide, would you hire me again, would you rec. me to your fam/friends.

I get about 1/2 who actually fill out and return them. Often there are praiseful comments, sometimes suggestions/critique. Sometimes just the 'rate me' part is filled out w/no comment. They can put their name on it or not--and when I give it to them, I tell them that even tho I might be able to figure out who it is, if they don't put their name on it then I will assume they would prefer not to discuss it later. Sometimes they do put their name on it, and this has led to some helpful discussions (for both of us, I think).

I give it to them at 6wk pp visit and tell them that there is no hurry but I hope they'll someday find the time, that their feedback is helpful to me.

I am happy to share it--how would I do that? Can I send an attachment here, or do I have to do copy/paste, or only send it to individuals?

I'm a doula and I'm creating one right now, and I've been a little lost as far as where to start...you've provided some really helpful ideas. I would love to see your questionnaire you've created! Hi there, I'm so glad you asked this question....I've had quite a bit of training in developing evaluations...though, I must admit, now that I'm creating one for my doula clients I'm finding it more challenging than I anticipated. One piece of advice I have for you is to mix up the questions - ask some yes/no, some Likert scale (rating), some that require more explanation, etc.

If you choose to create questions that use the Likert scale rating (for example, 1 to 10 with 10 being the best) then I would encourage you to make the scale 1 to 4 instead.

Example:
In your opinion, your doula stayed an adequete amount of time after your birth. 1- Disagree 2-Somewhat Disagree 3-Somewhat agree 4-Agree

Then the client will be forced to pick one side of the other...if you provided a middle option, then the person taking the questionnaire will be most likely to go straight down the middle.

Interesting about the scale you suggest and the reason for that....

My rating scale is based on 5--1 being 'best rating', 5 is 'worst rating'. I agree, 10 is too much to choose from. But I chose 5 because I wanted ppl to avoid the middle ground. Who knows what actually goes through their heads, but I have seldom gotten a rating of 3 (fair--middle ground) on anything. And in that case, it was only 1 of the answers out of about 25 total.
