NY mamas: vax record for Early Intervention?

We're going to have ds(3) evaluated for autism through our school district's special ed deparment. The woman I spoke with said we would need to "register ds with the district". Do they normally ask for a vax record? If so, the PHL doesn't mention vax "requirements" for Early Intervention, so I shouldn't need anything more than a short and sweet letter, right?

You mean an exemption?

Okay, I'm back The school did ask for a vax record, and surprisingly the Specal Ed Director didn't skip a beat when I interjected "Or an exemption?" He's going to be registered with the district to receive special education services, but not actually registered for classes (unless he qualifies for EI preschool, but that is yet to be determined). So I'm thinking something like this should be sufficient:

In accordance with NYS Public Health Law, Section 2164.9, the administration of immunizations, immunizing agents and PPD skin testing to our child, [name], is contrary to our genuine and sincerely held personal religious beliefs.

But if he does qualify for EI preschool, they'll probably want the SED form...so should I save myself the trouble and just submit the SED form from the get-go?
