We did a course of GV for five days, then stoppd because the "call poison control if ingested" label was worrying me. My pain got better while we did the GV, then came back a couple of days after we stopped and it's as bad as ever. What's the next step? I can barely stand this.

Gentian Violet failed - now what?

I don't know how you feel about taking medication, but Diflucan (Fluconazole) in addition to the gentian violet helped me enormously. I also made a major, major diet change and I am 100% sure that cutting out dairy and sugar is what has enabled me to keep the thrush away.
A great book for anyone with a yeast imbalance: The Body Ecology Diet
Look into the Diflucan if you're not opposed to it. It's not the first thing I would try by any means but it sounds like you've already tried a few things.

I'm in such pain, I'm not opposed to anything that will work. I've already cut dairy out of my diet because the baby is allergic, and barely eat sugar. I'll definitely look into the med. Is it prescription only?

It's prescription only and isn't a quick fix but I would recommend it too. You could try coconut oil, it has anti fungal qualities. You can rub it on your nipples after baby nurses. Plus it is totally edible so you know it won't cause oral cancer, which is why I hesitated about using genetian violet. My midwife warned about it being linked to oral cancer and I couldn't get past that.

Grapefruit Seed Extract was a lifesaver for us! I took it in capsules and did a wash for baby's mouth between feedings. I also used a topical ointment recommended on Jack Newman's website called the all purpose nipple ointment. That combination worked perfectly for us...
