Anyone know of companies who do not cover it?

In some states, Medicaid has dropped it, but that's the only one I know of.

Some do, some don't. Some plans do, some plans don't. And they change coverages all the time without telling people, so one plan that used to cover it might not cover it anymore. The only way to know for sure is to call around and find out.

Medicaid in the state of FL doesn't cover it and neither does my insurance plan. I have United Healthcare, which is quite a large company. I don't know if it's policy as a whole or if it specific to certain plans.

medcaid doesn't cover it in montana til a 'doctor says its 'medically necessary' I heard from the 'family 'support worker that boys got 'circ'ed because of infection & I just wonder if that 'was the simple natural seperation trauma going on with smegma discharge because if docs 'barely see smegma on circumcised males would that 'mean' they lack the knowledge of 'smegma as in what it really is and what is not' ?
