I can't help you on the bag, but I can tell you my DD2's placenta fit inside an oval casserole dish. It was probably around 12" x 8" (that's just a wild guess - we didn't measure).

I think it can look any way that pleases you. As for size, I should think that 12" x 12" would be the minimum. I'd likely go with a bit larger size and make it more oblong or rectangular. I think a nice longish oval shaped bag with drawstring would be good. Do you want it to fit snugly in the bag or can there be plenty of extra room? I would probably go bigger rather than smaller b/c too big would be better than too small. The "fits in a casserole dish" description is about right for what I have seen.
