Mildew stains?

Any ideas other than bleach? I use vinegar and tea tree oil and baking soda usually but eventually I always break down and use bleach to get rid of the stains.

I'd really like to avoid the fumes this time.

I believe the key is to figure what is causing the mildew in the first place and remove that issue.
