possible vaccine injury -- where do I start?

ok, this is my 6 month old niece we're talking about. my sil is open to suggestion, so i'm curious how to bring it up and where to look for further info...

basically since she was around 3.5 months old she has these little fits where her entire body goes rigid and stiff, for up to a couple minutes

it's gotten worse as the time goes by......she just had an MRI, but no results yet

you can get one of the reporting forms at cvs. they will have to pull it up for you in the pharmacy on their rxnet thingie. i don't reemeber the exact heading to pull it up under but if you go in when they're not real busy they shouldn't have to search too hard for it.
