have you searched the whole mothering site for rhogam? I've posted my feelings about this here a couple times--but do your own research! Don't base an important decision on other people's opinions and anecdotes. Not that they're bad, but it's always best to make your own decision after seeing all the evidence.
But anyway, I seriously seriously doubt that your MW would drop you b/c you didn't get the rhogam shot during your first pregnancy. This should not be a factor for her. If it is, maybe you should consider if she's the best person to attend your birth.
Good luck with your decision, and I'm so happy that you are thinking for yourself and not just going with the flow. Even though I choose to educate myself about things, it's often hard to go against authority--for me at least!
