Starting Infertility Treatment - Going Straight to IVF?

Hi, I'm Lisa. I've been a lurker for a long time, but I'd like to become more active now that I've got a definitive plan of action to talk about.

Background: We've been trying to conceive since May 2007. In February and March DH had two SAs and was diagnosed with poor morphology. On Wednesday we had our first appointment with our fertility specialist Dr. Winslow. He recommended up to three rounds of IUI and then continuing on to IVF if needed, and then he introduced us to his nurse Emma who who explained how it works, the medications and shots and timing.

Thought number one - it seems too easy. I don't mean the actual IUI/IVF process, but starting down the path. It felt almost anticlimatic, as simple as, "hey, you have a headache, here's an Tylenol." It seems like the decision should bigger, somehow. Did anyone else feel that way?

Thought number two - I'm blessed to work for a company that covers infertility (very unusual around here) but only up to $10,000. That covers, roughly, three rounds of IUI or one round of IVF. That being the case, the nurse Emma suggested that we might want to skip straight to IVF. According to their stats, there's 20% chance of pregnancy in every round of IUI vs. about a 50% chance for a woman my age to conceive with IVF. Given the odds and the financial realities, I'm inclined to go straight to IVF. I'd love to hear your thoughts on this. Any reasons I should do IUI first?

As someone who spent a LOT of money on three unsucessful rounds of injections with IUI, I would have skipped right to IVF if I'd been told the true costs and sucess rates up front. I see no reason why you shouldn't jump to IVF if you think it will work better.

I know someone who was in the same situation. She only had 10,000 for covered infertility and she went right to IVF. She is now pregnant with twins (due in August but I think they will be early). I guess it's up to you. Do you have any money saved if you run out of benefits? If you do you may want to try a few rounds of IUI first. If not you might want to skip on to IVF.

My diagnosis meant that we needed to skip right to IVF w/ICSI and I feel lucky that I didn't have to go through all the IUI cycles first. It might make sense to do one so they figure out what protocol works for you. The other benefit is it is less invasive but I always wanted the highest chance of success.

That's a tough one.

I'd wish for more money! LOL

I wish I could've done IUI, but I didn't have a choice but go straight to IVF, because of my condition. I hated IVF though and got really sick from it (moderate case of ovarian hyperstimulation that took me straight to hell and back). I hope to never do IVF again!

But, if I didn't have extra money to try for at least one IUI, I'd probably do IVF.

I know what you mean about it being a bigger decision. When I first got a diagnosis and they said "Clomid" I was shocked. Shouldn't we take more time about it before jumping into infertility treatment? It took me 4 months to finally go back and say I was ready to try the clomid. I tried to get my cycles going with diet and vitex, and it was only after I was sure that diet and herbs were basically making no difference at all that I was ready to move on to pharmacuticals.

I don't know anything about male factor, or if there is anything you could try to improve his counts naturally before jumping into treatment, but if you feel like it's too soon to put in that kind of investment you should wait till you're ready rather than feeling like the medical establishment is dragging you through your treatment, ready or not.

Lisa - that's a tough choice. I tried clomid, and when that failed was referred straight to IVF. So, I've never had the IUI option. However, the scary part about IVF is that there really isn't anything beyond it. I mean, this is the end of the line as far as fertility treatments. If it doesn't work, how would you feel mentally about going back to try iui's or about trying ivf again? Also, I know clinics have "shared risk" programs for ivf where you can get reimbursed or another free cycle if you don't get a positive. I don't know if the same deals are offered for iui's. Just somethings to think about... Good luck with whatever you choose!

My husband has the same problem (poor sperm morphology, 0-1% normal) and we went straight to IVF with ICSI. We were successful on our first try and our son is 13 months. If I had infertility coverage we would go straight to IVF again for #2, however because we don't, we are doing some IUIs this time around before an IVF cycle next year. Good luck!

New To Ivf World

HI, I AM NEW TO THIS IVF STUFF ALTHOUGH I HAVE DONE ALOT OF READING AND RESEARCH THE LAST YEAR... I HAVE FOUR CHILDREN FROM A PREVIOUS MARRIAGE AND HAD A TUBAL (THOUGHT I WAS DONE)
I GOT REMARRIED 2 YEARS AGO TO THE MOST WONDERFUL MAN WHO HAS NO CHILDREN AND WE DECIDED WE WOULD LIKE TO HAVE A BABY TOGETHER.... I THOUGHT MY TUBAL WAS OUR ONLY PROBLEM SO WE LOOKED INTO TUBAL REVERSAL, HOWEVER THEY ADVISED US TO GET MY HUSBAND CHECKED TO MAKE SURE HE WAS GOOD....SO WE HAD 2 TESTS DONE BOTH CAME BACK VERY LOW SPERM COUNT, SO HERE WE ARE LOOKING INTO IVF AND PROBABLY GONNA START IN THE NEXT FEW MONTHS
ANY ADVICE GIRLS ????

Hi Ladies,

Thanks for all your comments and advice. I told my doc today that we want to go straight to IVF. It's scary, but I think it's for the best. He said that we could start the process on my next cycle, which will be in about three weeks.

By the way, I forgot to ask my doc - does anybody know why IVFers have to go on the pill for a few weeks? I can't get my head wrapped around that one!

Yeah, that is a strange one. I was told 1) it was to make sure our bodies are in suppresion and 2) it was to help them time the IVF for the office. Apparently they only start so many women each week due to limits on the lab space and such. So...I was told I'd be on bcp's for anywhere from 10 days to 30 days. Somehow I ended up on them for 40. You might be cycling in August or September then. Some of the August ladies have already posted to the July IVF thread, so you can check that out. I'm sure another thread will be started. Good luck!

Due to severe MFI, we opted to go straight to IVF as well. Even though we have struggled through the process and haven't actually made it through a full cycle yet (though 2 canceled one's so far), I am glad on the one hand that we did not deal with all of the other options first, but it is a little scary knowing that we are at the last option.... I do have coverage for the diagnostics, but the actual IVF will be all on me.... so I know what your going through, but I probably would have made the same choice that you did.
