I've never come across an actual pattern, but this site has instructions and good pictures:

Never made one myself, but it doesn't look too hard to draft your own pattern from her pictures.

Here's another site that goes through it. It's similar to the other link but no serging involved. Looks very doable to me. Good luck!

I have made some prefitted's from
http://www.diapersewing.com/prefold_to_fitted.htm
like mentioned above and they work well. They helped allow a prefold that was too small for my ds to fit now.
