1. Your age at the time of the birth 21
2. Date of the birth August 2008
3. Sex of the baby Female
4. Weight of the baby 6 lbs 13 ounces at 18 hours old (no idea what she was at birth or how much, if any, weight she lost in those first 18 hours)
5. Number of weeks gestation at time of birth 40 weeks 1 day
6. Did you receive any "professional" prenatal care from a doctor or
midwife? If you did, how many visits did you have? No
7. Would you b econsidered low, medium, or high risk by a physician,
and why? Hmmm tough. Most likely medium due to circumstances with my first birth. They were user error though and not actually anything to do with my body. I'd say between low and medium.
8. Did you experience any complications during your birth? If so,
what? No
9. Did you transport to the hospital? No
10. Did the baby see a doctor after birth? If so, why? No. We did take her in at 18 hours old to my MIL's work (health department) and had the WIC office weigh and measure her. Turns out it was a good thing since we had to refile her birth certificate and needed to get another form of proof of birth (the original form was lost

11. Is this your first unassisted birth? Was it planned or unplanned? Yes and no. It was my second planned one but my first successful one. It was very much planned.
12. Please break your births down into categories:
# unassisted 1
# assisted hospital birth 1
# assisted homebirth
# assisted birth center birth
13. Did you have insurance at the time of your birth that would have
covered your birth? No. This was NOT a factor though. Had we had insurance we still would have done a UP/UC.
