1. Your age at the time of the birth 21
2. Date of the birth August 2008
3. Sex of the baby Female
4. Weight of the baby 6 lbs 13 ounces at 18 hours old (no idea what she was at birth or how much, if any, weight she lost in those first 18 hours)
5. Number of weeks gestation at time of birth 40 weeks 1 day
6. Did you receive any "professional" prenatal care from a doctor or
midwife? If you did, how many visits did you have? No
7. Would you b econsidered low, medium, or high risk by a physician,
and why? Hmmm tough. Most likely medium due to circumstances with my first birth. They were user error though and not actually anything to do with my body. I'd say between low and medium.
8. Did you experience any complications during your birth? If so,
what? No
9. Did you transport to the hospital? No
10. Did the baby see a doctor after birth? If so, why? No. We did take her in at 18 hours old to my MIL's work (health department) and had the WIC office weigh and measure her. Turns out it was a good thing since we had to refile her birth certificate and needed to get another form of proof of birth (the original form was lost
11. Is this your first unassisted birth? Was it planned or unplanned? Yes and no. It was my second planned one but my first successful one. It was very much planned.
12. Please break your births down into categories:

# unassisted 1
# assisted hospital birth 1
# assisted homebirth
# assisted birth center birth

13. Did you have insurance at the time of your birth that would have
covered your birth? No. This was NOT a factor though. Had we had insurance we still would have done a UP/UC.

This was the survey she sent me a couple months after my ds was born in jan 08. As far as I know she still wants them. I did email her recently tho, and haven't heard back yet. Will try again soon.

9. Did you transport to the hospital? No
10. Did the baby see a doctor after birth? If so, why? No

28
2. Date of the birth
March 13, 2008
3. Sex of the baby
Female
4. Weight of the baby
7lb 9oz
5. Number of weeks gestation at time of birth
39 (she was born 5 days before the 40th week)
6. Did you receive any "professional" prenatal care from a doctor or
midwife? If you did, how many visits did you have?
I did my own prenatal care AND also had free services from a midwife. They did no internal exams and used no ultrasound or doppler. I was too poor to purchase equipment, so I used their services for charting my weight, taking blood pressure, and checking fetal heart tones with a fetoscope as well as taking advantage of the free massage!
7. Would you be considered low, medium, or high risk by a physician,
and why?
My midwives were concerned about a small amount of sugar I was spilling in my urine, so a physician probably would have labeled me medium risk at best. I simply cut out sugar from my diet.
8. Did you experienceany complications during your birth? If so,
what?
It depends on what you'd consider a "complication." I had a small amount of hemorrhage which I cured by biting the placenta. Baby was born with cord around neck and I simply unwrapped her. She also had meconium in her waters which caused us no trouble.
9. Did you transport to the hospital?
no
10. Did the baby see a doctor after birth? If so, why?
In our state, pediatricians will not take on a new baby after 2 weeks of age, so I did take her for a quick well-baby visit at 2 weeks old. He just looked at her, declared her healthy and we went home.
11. Is this your first unassisted birth? Was it planned or unplanned?
This is my first unassisted birth. I have been planning it for 8 years.
