Could it be GER? At the end of my rope...

I see from Vanessa's post about Billie that several of you have experience with GER. Just wondering if any of you know of cases of GER where the baby *doesn't* spit up a whole lot?

We have such a mystery on our hands with Daniel. I'm exhausted and at my wit's end, really struggling. Here are his symptoms:

-- Very gassy and hence what I'd call "colicky". He grunts and groans and strains sometimes almost 24 hours a day, evidently finding it painful to move gas through his system.

-- He has had about 3 episodes that would qualify as colic: very hard, bloated abdomen with screaming, requiring many comfort measures to cope and to eventually help him pass the gas.

-- Here's the potential GER part: he is constantly sounding "throaty", as if there is liquid in his throat and he can't clear it. It's quite striking. Plus, he sometimes begins to choke and cough (loudly) out of the blue, making me think he's had some reflux. This can happen in his sleep.

-- He has trouble burping and when he does, maybe 20% of the time he spits up. He also spits up without burping, about 5 or 6 times a day.

-- He wakes up unhappy and too soon from most of his sleeps, clearly uncomfortable in some way. He often grunts and "throat clears" his way through all sleeps.

-- Nursing has become odd: on and off the breast, finicky, desperate to nurse but then backing off.... Anxious.

It could be the kind of colic that's essentially an immature digestive system that isn't coping well with anything I eat (and I'm eating a horribly restrictive diet because of all this and it's not really helping), but I wonder also if it could be primarily GER. I'm also looking into thrush, as my nipples are burning and itchy....

Have you tried block feeding? GERD usually doesn't involve gas, hindmilk/foremilk does. Sorry if we have already talk about this! I know there have been a few threads about this already, but I don't remember who.

I'm pretty sure that with DD we had a hindmilk/foremilk imbalance. It is slowlllllly getting better as my supply goes down.

That sounds like my first son who didn't have GERD but he did have ALOT of dietary sensitivities. If I ate wrong he screamed for hours and had very bad gas.
Because it was so many different things, when I took one thing out, it didn't seem to make a difference. Every health professional I spoke too said it was colic.
Finally at 6 weeks I went on an elimination diet (lots of info here if you search it). I ate nothing but dry cereal, chicken, rice and salad for 2 days and I had a different baby. All of a sudden he slept and was happy.

He ended up being sensitive to dairy, soy, tomatoes, any legume, broccoli, cauliflower, citrus fruit, garlic, onions, and anything with any spice to it.
He was a rare case, I haven't heard of many babies who had issues like him.

But have you tried changing your diet?

Yes -- I've eliminated everything enjoyable from my diet. I've been eating rice, fish, chicken, carrots, zuccini, rice milk, quinoa and spelt-flour bread/pasta for the past 3 days. He does seem to be a lot better, though I feel incredibly deprived.

But I think we're also seeing that there's some thrush. I wasn't sure, but it seems to be the case. I hope that once we treat the thrush, he'll be a little less fussy (aparently it can cause not only mouth pain but gassiness and fussiness).

Here's hoping that he's not sensitive to too many different things....

I'm not 100% sure that sounds like GERD...more like food intolerance or 'colic'. DD had GERD but also food allergies and beyond that just overall hated being a baby. It was a miserable time so I really feel for what you're going through. You can put a lot of energy into chasing down what is wrong - it really gets exhausting.

As far as food goes, it can take awhile to be certain of what is bothering him. Up to two weeks. If you think it might be helping even a little, stick with it.

Also thrush can DEFINITELY cause a lot of the symptoms you describe. It made my DD terribly gassy and fussy. Plus their little mouths can hurt so it makes them finicky while nursing like you describe.

Most babies reflux a little. If it isn't hurting them so much then usually it's not an issue. My DD never really spit up but when you could hear her reflux, she would scream a horrible painful cry. My DS now refluxes a little, I can hear it in the back of his throat. Sometimes he chokes a little on it - that's normal. But no screaming so I assume it's not bothering him so much.

I have had three babies with GERD - two silent refluxers (swallowed the reflux), one who spit up a lot. Three with food intolerances like JustVanessa had experience with (but my dietary things were different).

Jade doesn't seem to reflux, but I'm not counting it out due to my past experiences. With Graysen (my last baby), he was diagnosed with serious silent reflux after they did a x-ray photo shoot (he had to drink this gross, disgusting crud for it) through his gastroenterologist. It was very helpful in knowing the seriousness of it - we gave him meds to heal his throat for 2w and it helped so much - and he was on the reflux meds so no further damage was done...

I don't know how your insurance is, but are you able to meet with a gastroenterologist? We have a lot of the same gas and grunting and gas-passing-trouble (and nursing stuff like you too - but I've started to realize she does the "desperate and then back off" thing when she's tired, vs. hungry)... I think she/we may have thrush too... I think it's in my milk ducts now ). So much for the LC checking/not checking when I was there last week... I should have INSISTED. : sigh :

Anyways... him always sounding "juicy" makes me think it could be part of the trouble... but it doesn't sound like that's the only thing going on... HUGS!!!

Yes, I think there might be a few things happening. I definitely think he has some reflux issues. I just hate the idea of a barium x-ray to figure it out! But he often cries out painfully just after latching off the breast, and the throaty/juicy sounds are just so constant.

But yes also to thrush (I'm sure of it now) and probably also yes to a food intolerance or two. I think soy and dairy are the main culprits, and I only realized yesterday that the rice cheese I've been eating for the last 10 days in place of dairy cheese has casein (a milk protein) as the second ingredient!! Aaaargh!

Anyway, thanks for all the ideas, Mamas. Today has been a *much* better day for Daniel, who is really easy-going and peaceful when not in pain of some kind.
