GBS Positive..What to do?? Grrrr.....

Found out last Friday (2 weeks and 3 days before EDD) that my GBS test came back positive.

I just knew that it would because the whole trying-to-get-pregnant thing was filled with so many little "hitches" that now I expect there to be "hitches" (sad, I know...).

Midwives at my birth center recommend following the standard antibiotics protocol, but already gave me a waiver in case I decide to opt out. I feel really torn. I've read SOOOOO much about why doing the antibiotics could not be a good thing, and, of course, I also understand that there is a risk if I don't use them.

I was very interested to read about studies from Yale and Ohio State that say that the CDC's protocol uses way more antibiotics than is really needed to be effective. CDC says 3 grams penicillin to start, with 1.5 grams every 4 hours after. Ohio State did a study that seemed to show that just using 0.5 grams every 4 hours (including the first dose) is completely adequate. I was thinking of asking the midwives if they could just give me less antibiotic than what the CDC says. Has anyone here ever tried that?

What would you do? What are your stories? I know lots of people here would not use the antibiotics...and I know some people here have had friends (or themselves) whose babies had bad GBS experiences.

I am GBS positive and will be using a Hibicleans douche during labor. There are European studies that show it's just as effective as antibiotics and MUCH less invasive. Maybe you can ask if they're ever used that before. For me, it felt like a good 'middle ground.'

I refused to do the test with my midwives, and they were fine with it. So I didn't think it would be an issue for me.
Unfortunately, I did not get to give birth at home, as planned, but was transferred to a hospital instead. There the Dr. was not happy that I had refused the test, and told me because of that the baby would have to stay in the hospital for an automatic 48 hours. Dh and I were wanting to get out of there asap, so we could get back home to our other 3 children. Also, I knew that I would be in labor for a long time, and that I would be given antibiotics before long anyway because my water had broken several hours earlier. So, I told them to just go ahead and treat me as if I had tested positive, and they were happy to do so. I think I was given 3 bags of penicillin total, I'm not sure how much that is.
I decided that as soon as I got home I would begin taking my probiotics again to offset the antibiotics. I did that, and everything is fine. My baby is almost 2 weeks old now.

Here's my opinion for your situation, if it helps any (and please feel free to disregard). Since you decided to have the test done, I think you should go ahead and get the antibiotics (otherwise what was the purpose of the test, kwim?). You are likely very far along at this point, and likely have plenty of other things to worry about. I'm not sure that I would be in the mood to look at different studies and try to interpret those results and try to apply them to my situation. I trusted my midwives with my care. One of them stayed with me in the hospital throughout labor and birth, and I asked her about the vitamin K shot, newborn eyedrops, and the hep B vaccine. She said she allowed the vitamin K shot with her kids, but refused the eyedrops and the hep B vaccine. That was enough for me, and I did the same thing with my newborn. The hospital staff was fine with it. So, maybe just ask your midwife for her opinion, if you trust her with your care.

Planning to try to avoid PROM....

I'm in this boat now too. I will get the antiobiotics bc I will be in the hospital and I don't want to take any chances. What I'm really worried about now (since I'm 38+ weeks) is PROM. If that happens I know they will only "let" me labor for 24 hours before starting the "induction via pit" talk. Actually, that's when they'll want to implement it, so I need to be armed with info before then (if ctx don't start immediately we'll be using some natural induction stuff).

can help with strengthening the BOW prior to labor so they will be less likely to break prematurely.
I know Spark had a really PROM and resealed her BOW; I'm not looking to reseal, since it'll be labor, but to see if I can prevent PROM in the first place.

I tested positive. I was leary about testing but decided to for MY knowledge.

My midwives and I are comfortable following how the body goes. I'm past 37 weeks (one risk factor) and just hoping that my water doesn't break until well into labour.

If my water breaks then we'll go on the time frame. If it's 12 hours my midwife would really like to assess the situation, if I'm in full blown labour then I will be refusing unless it slows down closer to 16 hours...

If I'm not in full blown labour but progressing then we'll look at transferring sometime between 12 and 16-18 hours. (Time between 12-18 hours being another risk factor, if the water has already broken)

If I develop a fever (third large risk factor) we will also look at transferring.

You need at least 2 doses 4 hours apart to be of any benefit and this is why the time frame will be looked at.

I am also a possible penicillin allergy. If I wasn't then the midwives could administer the first dose at home. Since I am and IV penicillin has a larger rate of anaphylaxis we will be going to the hospital to try amoxicillin (I'm not happy about it but...)

Really I'm just hoping my water isn't broken for longer than 18 hours and I don't develop a fever

I tested positive, too...and despite the European studies and living in Europe the standard protocol is still to go ahead with antibiotics. A homebirth midwife I talked to here neither gives the test to everyone nor does anything about it if it's positive...no herbs, immune-boosting...nothing. Basically, what it seems to come down to is that there is some amount of risk in everything...not a lot of risk, mind you, but risk if you do nothing and risk if you follow their advice. I don't have pushy, interventionist care providers and everything has been very hands off and respectful so far, so I'm not planning to fight the antibiotics. It sucks, but I suspect I'll get through it. On my end, I'm juicing a lot of greens and eating very, very well and will of course start on probiotics.

It is a mega drag though! Grrr. I'm just choosing to focus on other things!

Actually, studies show that the amount of bacteria in the vagina goes down with just one dose. I read it on some generic "baby center" site - but then saw the study on the NIH website as well. I believe it said within just one hour!

The ideal they want is dose #1 5 hours prior to delivery & dose #2 4 hours later (1 hour prior to delivery), but even just getting one dose in you WILL reduce the amount of bacteria.

Not in your DDC, but my midwife is also a naturopathic doctor and she uses a golden seal suppository for a week (after 37 weeks because they cause contractions) and it has been very successful at killing off the GBS. You could find out about that and use that for a week and retest before labor starts. If you go with the antibiotics, just be sure to take a probiotic with acidophilus. Antibiotics during labor kills GBS, but is notorious for causing thrush in mother and baby. Good luck!!!

Yes, a 2007 study done at Yale found that risk of mom passing GBS to baby is gretaly reduced after just an hour or two of treatment.
