Have you signed but changed/edited the AAP's vax refusal form?

Did you have any issues from the ped about changing anything on the form? Did you make up your own form or did you just cross things out and add things to it and then sign it? Like, crossing out the part that you realize you are willing putting your child's health and life at risk. I want to cross that part off. Not sure how our ped will react to that though.

The AAP clearly states to Drs that the form can be duplicated or changed to suit the Dr's needs or the patient's needs (bolding and underlining done by the AAP).

Side note, the AAP also states to Drs that this form may make some wavering parents accept the Drs recommendations to vaccinate. That's nice...the form is part of the whole scare tactic.

I signed it like an idiot w/o reading it. When I asked about it (2 yrs ago?) they didn't know what form I was referring to.

I should ask again and re-read and cross out stuff as I see fit.

I crossed out the line about knowing I'm putting my child at risk, etc. Our ped had no problems with it (he also has no problem with us not vaxing). I read through the whole thing in front of him and said, "I'm not comfortable signing it with this line here. Do you have a problem with me crossing that off and initialing it before I sign?" He looked at it and said, "No, that's just fine! Shows me you know what you're doing."
