CDC an option for child care if not fully vaccinated?

This is a question for those of you that are military related. I'd like to be able to use the CDC on base once I go back to school. However, i'm selectively vaccinating my child, and I don't know if they have an exemption form or not. Does anyone know? Is it even a good idea to put my child in daycare at all if i'm not fully vaccinating? If not... what are my other options?

You use the exemption form for the state you're located in. They'll take it, but be prepared to run into at least one person who doesn't realize that they have to. DS went to preschool at the CDC on Ft. Knox, and a Ky religious exemption was all that was needed, no hassle at all. Other parents on other installations have had to fight, ymmv.
