1. How many children ages 5-18 in your family are currently

2. How many have received all recommended vaccinations for their

4. How many are unvaccinated? __ __
5. What is your e-mail address?
The study, which will be conducted by researchers at the University of Mississippi Medical Center,is in need of funding, and the preliminary survey findings could help secure the necessary funds.
