Vaccine exemption form?

Do you need to get a new exemption form from year to year.

We just gave one to my DD pre-k and at this time we are pulling her out of school she is not ready yet, so should i request the school return the form, or just let them keep it, because we will need a new one next year?

Good question! I would think you only have to apply for exemptions at the time of registering, which would be when the child is new to the school. If that is the case, then in your case I would think it would still count as long as you don't have to re-register.

As long as you remain in the same school district, the exemption should go on your child's permanent file and there should be no need to re-submit.

yeah this is just for pre-k so she wont be in the same school next year, should i ask the director for the form back so i have it for her school next year, so i dont have to go back to the health department in wait forever again. what do you think?

If it's considered part of her school records, they may refuse to give it back; no harm in asking though.
