I know it's not UES, but have you tried Whole Foods? I'm in South Jersey and we get non-homogenized milk there. You've got locations at Union Square, Chelsea, Tribeca, and Columbus Circle.

Whole Foods definitely does. The brand is Ronnybrook Farms. I bought it there plenty of times. I also saw the same brand in Grand Central Market, at the cheese stand.

Ronnybrook is unhomogenized and sold in glass bottles--you can get it in a few places. Check their website. It's not organic, if that matters, but it's grass fed.

Fairway by me has another brand, also in glass.
