Religious exempt question

For my state I have to write something about how vaccination is against my beliefs. Can I just simply state so and have it notarized? Or do I have to go into legal talk and details?

Also, are there no places on line that I can get all the other necessary forms? Or do I have to actually go to DHS or the health dept to get them?

I'm in Arkansas.

It sounds to me like you have to sign a notarized statement FROM the Dept of Health. So in other words, you go there, request an exemption and they give you a standard form that you must sign. It also states you have to complete and "educational component" prepared by DOH as well as sign an informed consent to refuse.
