PA school exemption question

I know I've asked before but just wondering if we have new people around. Next year my ds will be going to kindergarten. He has had many vax before I stopped entirely. Help me figure out how to fill out the paperwork. Do I include a notarized exemption letter or does that just draw attention? Should I just fill in the ones I''ve done and hope they don't notice? Any help or advice would be really appreciated!

I'm not positive, but I would guess the answer is to be honest about the vaxes he got, and then say you "found religion" in year 200X, and have since quit vaccinating due to your strongly held religious beliefs.

In OK I just did turned in an exemption form. My daughter was vaxed up to age 4.
