what if you just cross off the offensive language on the Refusal to Vaccinate forms?

long story short: we have been in NC temp and have been using a do in the area. At the last visit to get a clean bill of health needed for our move to europe one of the nurses freaked that my dd has not had any vax. She handed me a stack of info papers and the typical exemption form. It had the offensive language saying that I knowingly put my child at risk and blah, blah. The last one I signed (in CT where we lived, and where my dd has her more "perm" dr) did not have that language.
I did not sign saying that I had to check out the wording of the doc when we left the office, and I guess the nurse called CDC because I got a call that afternoon from the head nurse. She caught me off guard and so I didn't really understand why the nurse had called the cdc, but she wanted to inform me that they had said we would have problems getting our dd her passport because of the no vax status.... which is really incorrect, and when I informed her of that she hurriedly got off the phone.

Anyway, I am a little freaked that they called to report my dd, and will be going in to really get to the bottom of why she called in the first place.. and to establish my right to delay/ refuse vax. Can I just cross off the offensive language and give it to them to have in their papers? Should I trouble with printing an exemption form? It seems like an awful lot to go through since we won't go back as we are leaving in 3 days, but I also don't want to have the risk of having red tape because some ignorant fool decided to complicate matters.: I know the form is for the liability of the office, and that if I refuse to sign they can just refuse to see us...

That's actually a Refusal to Vaccinate form, which is different from an exemption form. A RtV form is NOT a legal document and no one can force you to sign it, although a doctor can refuse to see you/your child if you do refuse to sign. Docs like to have a signed RtV form in their files for CYA purposes, but they aren't required by law and really have no legal standing anyway. Personally I think they're just another way for docs to intimidate parents and take control.

I can't believe that they would "call the CDC to report" your child for not having any vaxes. If the CDC had to field such calls every day, they'd never have time for anything else! I'm not even sure they would take such a report. I wonder if this was but another scare/intimidation tactic by a manipulative nurse.

Take back that control, and refuse to sign. Instead, offer to give them a short statement (prepared and signed by you) refusing vaccinations, or a copy of a notarized school exemption. Do not be bullied - and see this thread for my recent experience.

oops- you are right. It is a refusal to vax form. What does CYA stand for?
