getting vax exemption form back from school

well my DD refused to go back to preschool, so she is not going, this year, but i was wondering since next year she will be attending a different school i will need another exemption form, so do you think the school will give me back her exemption form?

Or do you think I will need to go through this whole process of goint to the health dept waiting three hours to obtain the form for next year?
any advice on how to approach this, to get the school to give us the form back, theya re pretty pissed that we pulled our DD out, since they were expecting the tuition on our end.

also how do we go about getting in the shots out program so my DD vax status cant be tracked, can we fill an online form out or do we have to go back to the health dept again?

I don't think you get a new exemption every time you switch schools. They need to transfer the records to the new school when you register.

I would agree with this too. Also in our state, once the form is signed you never need one again, ie each school year or what not.
