signing a form

my both sons are vaccine free, but we still go for checkups. A few times I signed a form that I'm refusing vaccination. I didn't really pay to much attention to the document, just signed it and left. And here I read that I shouldn't do it.
Please tell me what are the consequences? What is wrong with signing the papers? In 2 weeks I have another 6 months checkup and I know they will ask me to sigh again.
Please, give me some good advice. Thanks!

it often contains language such as "I realize I am endangering the health of my child" ....

grounds for removal from parental custody, technically, by the state agencies.

I read that they can take your children for 48 hrs. and investigate you.
next time tell them you'll take the form home to read.

Be aware that our ped practice (whom we have been with for over 18 years) just "discharged" my children from their care b/c I declined to sign their Refusal to Vaccinate form.

These forms are for THEIR OWN cover-your-ass purposes. You get no such coverage ... I doubt they'd sign a form acknowledging that they are possibly endangering your kid by administering vaccines, and that they would take full responsibility for any harm that came to your child b/c of vaccine administration.

So what should I do now? Can I ask them to give me back those papers that I've already signed? Can they really take the kids away from you? It's unbelievable. I think I'm going to go with my husband for next checkup.
Thanks!

Unfortunately, no. They are part of your child's medical records and it is illegal for anyone to remove them.

I would ask to see the previous forms. If it doesn't say any of the "endagering" language, I would just create your own letter to be added to your child's medical records that indicates that 1. you do not vax due to religious beliefs. I would go on to say that you have been educated about the risks and benefits to vaccination. If it does contain "endangering" language, I would also include that this addendum voids any previous waivers or signatures included regarding vaccination and that you are not endangering your child by following your religous beliefs wrt vaccination.

Here is a form you can use. Copy it, sign it and give it to them.

As for the forms that are in the files already, forget them. There is not much you can do about them.
They can not take your child away from you because you broke no laws. Vaccines are not a law. They are only mandated for school/day care.
As long as you and dh agree that is how you want to take care of your child, there will not be any problem. The problems comes when one parent changes his/her mind. Then the court comes down on the side of the parent who wants to vaccinate.
