Vax exemption form for public schools

When I called the school that we will most likely enroll our children in, I asked about vax exemption and the person I talked to said there was a form to fill out. Are any of you familiar with a form for public school enrollment in regards to vax?

There is no form in NC.

A written statement must be maintained in the student's record containing, at a minimum, the following - student's name; parent(s), guardian's or person in loco parentis statement of bona fide religious objection; and parent(s), guardian or person in loco parentis signature and date signed. (If a student is at least 18 years old, his/her statement and signature are required.)

N.C. Gen. Stat. § 130A-157 (2006)

§ 130A-157. Religious exemption

If the bona fide religious beliefs of an adult or the parent, guardian or person in loco parentis of a child are contrary to the immunization requirements contained in this Chapter, the adult or the child shall be exempt from the requirements. Upon submission of a written statement of the bona fide religious beliefs and opposition to the immunization requirements, the person may attend the college, university, school or facility without presenting a certificate of immunization.
