Personally, I would give them the absolute minimum you can get away with. It doesn't sound like they are asking you to
name your religion or show actual
proof that it is against your religious doctrine, so you shouldn't be expected to provide that. Otherwise, they would be venturing into religious discrimintation territory by witholding benefits unless you belong to a specific religion, KWIM? I would start out with a simple notarized letter, stating that the practice of vaccination conflicts with your
personal religious beliefs. And if they continue to ask for more details, maybe ammend to your letter to explain that while you are not a member, you hold similar beliefs to that of say... Christian Scientists, for example.
