...how do you get clients?

I taught childbirth classes for homebirthing, and years ago (1983-6) I had lots of trouble getting clients to get classes going.

Most midwives taught their own classes.

If I taught hospital classes, I would get lots of clients from the hospital, but that is not me.

I teach for all kinds of clients, home, hospital and birth center, however it is mostly the more pro-active women that call me.
Word of mouth seems to have gotten me this far- occasionally I run a small paper ad- like every couple of years. I do PR for myself by getting a press release ( KeysMama has completed an updated for her childbirth educator certification...etc)
Also, I have cards that I pass out, and local agencies ( WIC, Healthy Start, offices, clinics) have my cards and will refer to me.
I also do some free work on occasion for PR like holding a "free birth options" class at the library and get that in the paper.
Cards at libraries, bookstores, health food stores, etc.
I teach about 1 class per month privately.
