Nashville, TN - holistic practitioner?

I am looking for a holistic practitioner to take over my care and treat my myriad of medical conditions, ranging from asthma to diabetes. I haven't a clue where to start looking, and I hope someone here can recommend a practitioner, such as a naturopath, homeopath, etc. I'm willing to travel to Nashville (I live in KY) to find someone.
