3 year old with infected foreskin - Mothering Forums

I'm new to this website and am so glad I found it! My 3 (almost 4) year old is intact and I didn't realize we would be encountering problems with that decision. 2 years ago his penis got infected (we think because of swimming in our non-chlorinated baby pool). We had taken him to the doctor and he was given antibiotics. It cleared up and we haven't had any problems until this past weekend. Tuesday morning he was pooping and I came in to help him wipe and saw that his foreskin was pulled back and it was red and swollen. I took him to the ER and he was given antibiotics. We are 3 days in and there still isn't much change. The foreskin is still pulled back and it is really swollen and it hurts to touch. It almost looks like an inner tube around the head. I also found a little yellow-ish patch right on the tip. The ER doc told us that if it doesn't clear up, we have to take him to a urologist. My worst fear is that he will have to be circumcised. I can't even imagine making him go through that at this age. Has ANYONE ever gone through this? Is it something serious that needs medical intervention or will the antibiotics take care of it? Please help!

Anyone have any advice for this mama?????? I have no idea what it could be but how stressful*hugs* mama. Try tostay strong and give the antibiotics a few more days!!!

This child obviously has paraphimosis. He has pulled his foreskin back and now it is stuck behind the glans penis.

 

This interferes with blood circulation in the penis and the result is edema, which is what causes the swelling. There is no reason to believe that infection is present.

 

The proper first aid is to squeeze the head of his penis between thumb and fore finger to force the blood out of the head so as to reduce the size at which time the foreskin may be brought forward. If that is not possible, and it may not be possible because of pain, He should be taken to a DIFFERENT emergency room. It is quite distressing that this ER doctor apparently does not recognize a clear case of paraphimosis. Antibiotics will NOT help paraphimosis because this is NOT an infection.

 

Paraphimosis is a medical emergency because circulation is impaired. The emergency is over when the foreskin is brought forward to its normal position.

 

This child does not need a circumcision, although some foreskin-unfriendly doctors might suggest it.

I hope you got help for your ds. paraphimosis is an emergency and severe damage can result if it isnt fixed ASAP

I forgot to mention that it may not be possible to reduce this case of paraphimosis without local anesthesia because of the pain involved.

In the meantime, however, it's crucial that the mother knows this is a medical emergency.  The tourniquet action you mention can cause gangrene, so this boy needs to have the fixed stat! The doctor who saw her son was uneducated. The mother needs to stop giving her son antibiotics. Then, because she says it hurts when his glans is touched, I'm not sure she'll be able to reduce the swelling enough to bring the foreskin forward. She can try gently squeezing the fluid from the glans (which is like a sponge) or applying ice, but this has gone on for so long and the boy is obviously suffering that he needs to be seen by a medical professional who can deal with him appropriately. She needs to call her son's pediatrician and get a referral to someone who is skilled at reducing a swollen foreskin without ever mentioning the "c" word!

Perhaps you could cut and paste this for me...

You have been doing the right things. You did right to protect your son from circumcision. Unfortunately, he pulled his foreskin back and it got caught behind the head. This is not your fault. Sometimes kids just do that.

 

You did right to take him to an emergency room. Unfortunately, you got a doctor who does not know what he is doing.

 

You did right to come to Mothering for advice. You have received good advice. I hope you are following it.

 

We are all interested in your son's welfare, so we would like to know how he is doing.

If nothing else, please contact Marilyn if this is not yet resolved.  It can be resolved but it needs attention right away!  Oh, & please let us know how he is, when you can.

Thank you all soooo much! It was definitely paraphimosis. I took him to a urologist and told them they needed to do everything possible before circumcising him. They put him under and was able to pull the skin back without cutting him at all. So wonderful! He's back to himself now. Again, thank you!!
