I have been diagnosed with GD in week 27 of my pregnancy. At what point in the pregnancy is it typical for midwives/doctors to start medication?

There is no specific point. Medication is based on your blood sugar control. I assume you are doing your sugars four times a day? The usual guideline is two sugars higher than the target maximum in one week = medication (usually insulin). However there is usually some flexibility in this, especially in the early weeks after diagnosis when you are learning your triggers.
