I know this topic has come up from time to time and I came across this idea from my cousin, who is an artist. Everything from a greeting card to small or large prints of her painting of a pregnant woman.
Another thing I did for my midwife a few years ago is make a photo book for her of pictures of my daughter's birth and the story. She loved it.

I've never really understood the need for gifts for the birth attendant(s). Maybe I'm just crazy, but it seems like paying $3,000 - $5,000+ is a big enough gift! On top of that, a thoughtful thank you card is probably all I'd do.
