Birth certificate application info wanted for California - Mothering Forums

Does anyone have a birth certificate application handy, or have memorized the contents on it? I vaguely remember with my first child that the mother and father's social security numbers and race were optional, but I'm not sure.

You could call any county Registrar office on Monday and ask them. I don't have one here at home, but I have then in my office. Is Monday too late? In California, if you are not legally married you have to fill out a POP (Paternity form) that is filed with the county, or the father's name cannot be put on the BC.
