I think they start you on that dose to see if it works for you, then increase as needed. I've had some success on the 40 (as of 6 days), but would love to be making more milk. I'm pumping w/ double electric pump 6X daily, nursing at least 8X per day, and on fenugreek as well. Still only able to pump approx. 1.5-2 oz in 20 mins.

I'm taking 80-100 mg a day plus 9 fenugreek capsules a day. BTW, where are you getting it filled? Did you find a compounding pharmacy to make it? Or are you ordering it online? Is your ins. co. paying for it? It's so darn expensive!

My midwife prescribed it and I had it made at a compounding pharmacy, but it was like $65. I now get it at globaldrug.tv. Only $20 for 100 tablets. (plus shipping of course)
I take 12 fenugreek/day and have tried the oatmeal, tea, you name it.
MW's are great with breastfeeding, but may not be totally up on all the latest info in lactation. Therefore, she likely went with the manufacturers label directions on domperidone which is " 10 mg three to four times daily. If necessary this dose may be doubled after two weeks if an adequate therapeutic response is not attained with a maximum daily dose of 80 mg."
So, follow directions and common LC advice that you read on reputable sites.
