I have a facebook friend who recently UCed and is trying to get the birth certificate. She says the forms she was given ask for the "Certifiers name". I am not in her state and my paperwork never asked that. Has anyone else filled out this form and know what she is to put for the "Certifiers name"? Thanks.

That's odd. She should probably just call the department of health or wherever she got the forms from, they should be able to tell her. They were quite helpful in answering all of my questions when i was getting the birth certificates for my twins.
