1. Your age at the time of the birth - 25
2. Date of the birth - 2/5/2008
3. Sex of the baby - F
4. Weight of the baby - 6lbs 1 oz
5. Number of weeks gestation at time of birth - 36 even by mw's dates, 36.5 by mine
6. Did you receive any "professional" prenatal care from a doctor or
midwife? If you did, how many visits did you have? - yes, many, I forget how many
7. Would you be considered low, medium, or high risk by a physician,
and why? - Probably medium, I had HG and officially had a number of other things wrong with me, although I didn't consider them issues (i.e. failed the GD 1 hr test (regular blood sugar counts were fine, I checked myself), I had lots of Braxton-Hicks throughout, etc.
8. Did you experienceany complications during your birth? If so,
what? - nope, none
9. Did you transport to the hospital? We went afterwards, we had the baby, chilled as a family for a little while, and then took a casual ride to the hospital (for DH's peace of mind) and ended up staying overnight : but they didn't really do anything other than observe us (and charge our insurance ha)
10. Did the baby see a doctor after birth? If so, why? - yes but only for a physical once-over so we could leave the hospital, no shots or tests
11. Is this your first unassisted birth? Was it planned or unplanned? - it was our first UC and it was planned, although if I had felt squicky at any time I was OK with going to the hospital too
12. Please break your births down into categories:

# unassisted - 1
# assisted hospital birth - 0
# assisted homebirth -0
# assisted birth center birth - 1

(I put down assisted birth center even though the birth center was IN the hospital...)

13. Did you have insurance at the time of your birth that would have
covered your birth?

Yes, although it didn't cover homebirth. If it had I *might* have gone down that route.

1. Your age at the time of the birth
28
2. Date of the birth
4/05/08
3. Sex of the baby
Male
4. Weight of the baby
6 lbs.
5. Number of weeks gestation at time of birth
37 weeks
6. Did you receive any "professional" prenatal care from a doctor or
midwife? If you did, how many visits did you have?
Yes - one visit with a hb midwife, just testing blood levels, urine and bp.
7. Would you be considered low, medium, or high risk by a physician,
and why?
Low, was low for my first birth w/ ob care
8. Did you experienceany complications during your birth? If so,
what?
My placenta was very low, it may have been partial previa, I never had an ultrasound. I always felt it would be okay, I listened to my placenta at home and most (if not all) of it was off the cervix. But there was blood in the amniotic fluid when my water broke right before I pushed ds out. My placenta took about 7 hours to deliver, which made me nervous.
9. Did you transport to the hospital?
No, but I did ask the mw's assistant to come check me out. She had been doula at my dd's hospital birth
10. Did the baby see a doctor after birth? If so, why?
Yes, pediatrician visit to get him on record. Later I took him back to the mw for PKU and a vit K shot as he was having weird meconium that suggested his intestines needed some help with clotting/closing up.
11. Is this your first unassisted birth? Was it planned or unplanned?
First unassisted & planned that way
12. Please break your births down into categories:

# unassisted - 1
# assisted hospital birth - 1
# assisted homebirth - 0
# assisted birth center birth - 0

13. Did you have insurance at the time of your birth that would have
covered your birth?
Yes it would cover a hospital birth, not hb. We could afford an out-of-pocket homebirth as well. Insurance co-pay was 2000, hb mw was 3500.

6. Did you receive any "professional" prenatal care from a doctor or

7. Would you b econsidered low, medium, or high risk by a physician,
and why?
Not at all, unless you consider really quick labour, like, no way I'd ever make it to a hospital quick

No complications at all, though he was a Frank/Pike Breech
9. Did you transport to the hospital?
No
10. Did the baby see a doctor after birth? If so, why?
The Midwife came to check us after the birth, and we took him in for x-rays of his hip when he was a week old due to sever bruising and some swelling on his hip/buttock due to his presentation

It was my first, it was planned
12. Please break your births down into categories:

I'm not sure where to put my first birth, technically it was in a hospital, but as it was a holiday there were no doctors or anesthesiologists on staff that night, with only the midwifes and Nurses present and no chance of pain meds it was more like a birth centre really...

13. Did you have insurance at the time of your birth that would have
covered your birth?
Yes, I live in BC so even HB MW's are fully covered

8. Did you experienceany complications during your birth? If so,
what?
Nothing major. I had a water leak a few days before the birth, but it resealed. My older son was 9 pounds 15 ounces and I knew this one would be bigger, so it was nice not to have the "big baby" scare tactics and pressure.
9. Did you transport to the hospital?
Nope.
10. Did the baby see a doctor after birth? If so, why?
We have a fabulous Ped we love who's supportive of our choices, including homebirth. We went in at 6 days PP and did a quick looksee and she wrote us a letter.
11. Is this your first unassisted birth? Was it planned or unplanned?
Yes, very planned.
12. Please break your births down into categories:
#1 & #2 planned assisted hospital birth (#2 was a birth center attaached to a hospital - same diff)
#3 UC

13. Did you have insurance at the time of your birth that would have
covered your birth?
Yep.

Sarah
aspiring midwife Mama to Declan - 7/14/00 and Bastian - 11/20/07.

what?
none whatsoever

13. Did you have insurance at the time of your birth that would have

1. Your age at the time of the birth 21
2. Date of the birth August 2008
3. Sex of the baby Female
4. Weight of the baby 6 lbs 13 ounces at 18 hours old (no idea what she was at birth or how much, if any, weight she lost in those first 18 hours)
5. Number of weeks gestation at time of birth 40 weeks 1 day
6. Did you receive any "professional" prenatal care from a doctor or
midwife? If you did, how many visits did you have? No
7. Would you b econsidered low, medium, or high risk by a physician,
and why? Hmmm tough. Most likely medium due to circumstances with my first birth. They were user error though and not actually anything to do with my body. I'd say between low and medium.
8. Did you experience any complications during your birth? If so,
what? No
9. Did you transport to the hospital? No
10. Did the baby see a doctor after birth? If so, why? No. We did take her in at 18 hours old to my MIL's work (health department) and had the WIC office weigh and measure her. Turns out it was a good thing since we had to refile her birth certificate and needed to get another form of proof of birth (the original form was lost
11. Is this your first unassisted birth? Was it planned or unplanned? Yes and no. It was my second planned one but my first successful one. It was very much planned.
12. Please break your births down into categories:

# unassisted 1
# assisted hospital birth 1
# assisted homebirth
# assisted birth center birth

13. Did you have insurance at the time of your birth that would have
covered your birth? No. This was NOT a factor though. Had we had insurance we still would have done a UP/UC.

6. Did you receive any "professional" prenatal care from a doctor or

9. Did you transport to the hospital? No
10. Did the baby see a doctor after birth? If so, why? No
11. Is this your first unassisted birth? Was it planned or unplanned? This was my first UC. It was planned. Originally we were going to have a MW assisted birth at my husband's request, but then we mutally decided to have a UC because we were moving out of state. 9. Did you transport to the hospital? No
10. Did the baby see a doctor after birth? If so, why? We had a home visit when he was 5 days old. We did it for the birth cert. Baby checked out perfectly!
