I'm stumped! Please help me identify this persistent, yucky rash - Mothering Forums

DS (now 6 mo) has had the most crazy rash... no one can figure it out... even the docs. Looking to you smart ladies for some answers!

The rash started in his diaper area when he was 2 mo. Thought it was from his overnight cloth diaper. It never went away for more than a day or so, but it varied in intensity. It was bright red like a "raised sunburn", and little dots (but not raised) on his stomach (up to his belly button), groin, and inner thighs.

At 3 months, it started spreading to his trunk - little bumps and bright red blotches. The pedi thought it was yeast... but the stuff on the trunk doesn't respond to the GSE I was using (the diaper area responds well to diluted GSE). The rash on his trunk doesn't respond to diaper creams or ointments, either. It itches him, he uses his hands to rub at it when the clothes/diaper come off (but not when he's dressed).

In the last month, it has spread even further. Now it's red pimply dots on his trunk, armpits, thigh creases, neck, and they just started on his back. Some bumps are big, some are tiny. Some of the pimples have big red angry blotchy things underneath them, too. They're not oozing or crusted. But they're bumpy. Sometimes they look really angry, sometimes not. It changes daily.

Has anyone experienced a rash like this before? I'm trying to figure out what the cause might be? Some thoughts are:

He's not pooping every day (but this shouldn't really matter right, since he's exclusively breastfed?)
Contact dermititis?
Food allergy to something I'm eating

Would love your thoughts... I have tried lots of things, but nothing is working on his trunk area. The only thing that's rash-free consistently as of the last few weeks is his diaper area, go figure!

DS had a somewhat similar persistent rash starting around 5 months that turned out to be eczema related to food allergies (via my milk, since he was still exclusively breastfed). At a couple points the rash started looking like pimples, and that was impetigo - a skin infection that was able to gain a foothold because of the eczema. We were given steroid and antibiotic creams to clear it up, then we did blood and skin tests to identify his allergies. After eliminating all the problem foods (wheat/dairy/eggs/nuts) the rash disappeared. He still has sensitive skin and gets a rash if an area is too hot/damp, especially under his arms and around his thighs.

Testing for food allergies or trying an elimination diet might be worth it if can't identify any other triggers.

My 5 month old has the same thing! At first I thought it was because we used disposables at night so he is now an all cloth diapered baby. Tamanu oil was working but that stopped. So I tried an herbal diaper rash cream and I think that actually made it worse. Then I tried some anti-fungal cream and it lessened the redness a bit but not by much. I let him go coverless today and the redness went down and I started to take acidipholius (not sure how you spell that?!).

He doesn't seem to be in an discomfort but he does grab the irritated areas when I am changing him. ANd I think it is spreading to his waist now?!

Bright red and pimply sounds like what our DD's pediatrician warned us of what a yeast infection looks like, but it could be a diaper rash or allergy (food or contact), too.

Have you tried stripping your diapers yet?

Also, do you notice it change (on his abdomen) depending on the types of clothes he wears? It could be a sensitivity to a type of fabric. If there are metal snaps on his outfits, he could even be allergic to that (nickel allergy).

If it isn't yeast, then I'd think food allergies, but I wouldn't rule out yeast either at this point.
