I am not as familiar with the state regulations as the others here are -- but my understanding is that anything short of being totally up to date according to the state/CDC's schedule would require a waiver.
What state are you in as the regulations and waivers vary by state. Certain states allow for a philosophical belief waiver and in those states it's easier to do some vaxes and not others. But if you have to do a religious waiver because the philosophical one is not available in your state, you will probably need a plan for selective vaxing.
|Q: How do I get a religious or philosophical exemption for my child to not get any immunizations?
Answer: Religious exemptions can be granted for children "on the grounds that such immunization conflicts with the religious tenets and practices of a recognized church or religious denomination of which said parent or guardian is an adherent or member" in accordance with Arkansas Statute.
E-mail us to request the exemption form.
PLEASE NOTE: The form requires a signature and seal of a Notary Public.
Complete and mail to:
Arkansas Department of Health
4815 West Markham - Mailslot 48
Little Rock, AR 72205.
PLEASE NOTE: Incomplete forms will be returned for completion. A new Exemption Form Must Be Completed Every School Year.
