Need religious vaccine exemptions - Mothering Forums

i have a medical note from a dr which I could present to DS's school, but I have to update it yearly and am afraid about what if the dr stopped writing the note. So I think we'll do religious exemption (though it does scare me from a CPS standpoint).

We're moving from TN to OH, and I need a TN exemption to present to them now w/ all the registration info and then an OH exemption once we move there. Do I just type something up and have it notorized, or is there something more official?

I know OH has a personal exemption, but the school is private and only accepts religious or medical.

This is what I did. I'm in Ohio, and we turned ours in today. We didn't have any issues with it, thankfully.
