My sil is wondering how to go about getting a religious exemption in Tenn. Do you make an appointment at the health department? She's in Nashville if it makes a difference.

No...it looks like you file it with the school

Tenn. Code Ann. § 49-6-5001 (2006)

(1) It is the responsibility of the parents or guardian of children to have their children immunized, as required by subsection (a).
(2) In the absence of an epidemic or immediate threat thereof, this section shall not apply to any child whose parent or guardian shall file with school authorities a signed, written statement that such immunization and other preventive measures conflict with the parent's or guardian's religious tenets and practices, affirmed under the penalties of perjury.

You dont need an appt at the health dept you just drop by and ask for one. They go get it you fill it out then turn it into the school instead of the vax sheet.

They have a new one now that isnt green it is just white and it dosnt have the same layout as before.

I am near Knoxville by the way.

Just looked at that second link in pdf form and that is the form you get from the health dept. so dont even bother going there just print out that paper and fill it out. Wish I had found that when I was looking I could have saved myself a trip

The old form you had to have it noterized. That was back when dd first started school in 06.

Thanks! She homeschools her kids, so she just wants the exemption forms as a cya type of thing. I'll send her the link.

she can also go to the states website and in the search bar type in immunization exemption and it will pull up a letter that she can print and sign.
thats what i did for my daughter and we live in TN
