Yes they are. You can find them on the website of the health department. If you don't have an exemption they require pretty much everything on the CDC schedule (Dtap, ipv, hepb, prevnar, hib, even flu!).

Connecticut Department of Public Health
 Religious Exemption Certification Statement
According to State statutes (Connecticut General Statutes Sections 19a-7f and 10-204a), no child may be
admitted to a licensed child care program or school without proof of immunization or a statement of
exemption. To claim a religious exemption on the basis that the immunization would be contrary to
religious beliefs of the child, the parents or guardians should complete the following statement and return
it to the school or child care facility.
To Whom It May Concern:
As the parent(s)/guardian(s) of __________________________________________,
 (Name of student)
I/we hereby assert that the immunization of this student would be contrary to the religious beliefs of this
child. Therefore, this child shall be exempt from the required immunizations under Section 10-204a of the
Connecticut General Statutes and shall be permitted to attend a licensed child care program or school
except in the case of a vaccine-preventable disease outbreak.

Yup - just the form.

I had no issues personally. In fact the preschool supplied the form for me. I just registered S for Kindegarten in the public school and they supplied the form for me as well, but you can also download it if they don't
