Maybe it is different where you are, but here we don't need a dr. signature. My first exemption forms, I just went to the local health dept and they made me read a long paper about vax that was supposed to change my mind, but made me just be more against it. I had to initial the paper on the front and back and then fill out the card with child's name and check all vax I wanted exemption from and then check one of the reason why I wanted to be exempt and sign. They told me to keep the card like I would my vax record. When my dd started school, that is what I showed the school. Eventually the school wanted a new one every year, but instead of going to the health dept, they had the cards in the nurses office and now every year I just refill out. I never need any dr signature or anything. Although perhaps the nurse would sign it later, as she would keep the card. Either way, I didn't have to go to the dr for anything.
I am not in the same state, but you might check and see if you have to have the signature or not. I am sure it probably varies from state to state....but it is worth checking into =)
