You just print it out and sign and date the bottom of page one. That's it. You may furnish previous vaccinations/dates if you want or leave it blank. Even if you had previously vaxed, you are still able to claim religious exemptions for all or some vaccinations. First off, you may have changed religions. But there are also cases where, for instance, if you object to abortion (just a popular objection), you may not want to get those shots that have aborted fetal tissue. Or perhaps you object to pork products. Many providers (health and daycare) aren't familiar with exemptions and so pass on incorrect information. Many don't even realize that we have a choice. The one word that may give you a problem is 'any' in the statement. If you are worried about it, I'd leave off any vaccinations that you do want to get. Or get the vaccination, note it on a new form and then sign at a later date. At the date you signed, you may
have bonafide religious objections to /ANY/ vaccinations that you previously didn't.

You don't need to discuss your religion with them. Just remind them that you enjoy religious freedom. Please also note that there is no place on the form for a religious leader to sign and no where does it request a letter. Don't fret
or worry about it. If they ask you to fill out their own form, explain that you are providing the form required by Maryland and that they cannot infringe on your religious freedom by asking more of you then the law requires. If they don't like the law, they can go lobby somebody.
