Couple of quick ?'s from TN- - Mothering Forums

1- DD will be starting school in Aug and I need to find a vax exemption form to bring when we register. (I'm not sure that they'd even know what I'm talking abt if I asked for one there-) Can someone pls link me an easy clearcut form letter that's good for my state?
2- I completely forgot abt the stupid physical they require for school admission. I know I can expemt vaxes, but can I also exempt a physical? If not I have to make an appt immediately-

I know that in CA a parent can waive the physical and the TB test. I am not sure about TN. Look at the form and see if there is a check off for exemptions from the physical and the TB test. Or simply write a letter that states you are not getting a physical and attach it to your vax exemption.

Here in HI, the school had to give me a form to fill out for an exemption.It took a couple of days for them to produce it but they got it to me.In the meantime DS was allowed in on a provisional admission.I had to sign a form saying that he was going to get his physical and (presumably vaxes too) by a certain date.You might want to ask the school about that.I had made the appt for his Dr visit too late for the first day of school.

Just an FYI for anyone ISO: I found a letter on the vaclib site. A (currently broken- I've reported it.) link to the necessary .pdf exemption form can be found http://www.kcs.k12tn.net/index.html. (Just go to the site index, click on Forms Online and search vaccination and it'll pop up. Can link directly bc it's down.) It can be also faxed to you if you call 865.594.3735, or it can be picked up downtown; but it cannot be emailed.

I've left a message for the director of the vaclib re: the possibility of physical exam exemption.

If anyone knows anything more, pls feel free to post. I've never dealt w/this before b/c I naively vaxed dd1 on a selective and delayed schedule. She had all the necessary vaxes to enter school though.
