My kids need to get physicals before they start school and we don't have insurance currently. My mother in law works at a doctors office (in alabama, we live in florida but are right over the state line) and I could get their physicals at her office for free or for very little probably. However, my mother in law is not aware that we do not vax and I'm not keen on filling her in. They just aren't very...accepting and I have enough going on in my life right now. Here's my question: if I took my girls to the doctor she works for, would he request an immunization record? And if so, if I told him I left it at home/forgot it, would they still be able to say, call the health department or a previous doctor to try and find them out? Just trying to cover my bases here. Thanks for any responses!
I don't know about Florida, but Alabama has mandatory vaccine reporting- if your child gets a vax, the Dr has to report it to the state and the parent is not asked for permission nor can they opt out. I don't know if a doctor in Ala could check the Florida registry to find out what vax they had.

To Whom it May Concern,
I am religiously opposed to school physicals. Please excuse my children from this requirement.
Here is the Statute:
Florida shows that the exam exemption must be on the immunization exemption form. That form doesn't seem to be available on line, so I'm not sure where on the form it goes - maybe you just have to write it in. I would include the actual statute. My school nurse had never seen a physical exemption before and that was helpful.
|Any student will be exempt from the requirement of a physical examination upon written request of the parent/guardian stating his objections to such an examination on religious grounds. This exemption request must be presented on DH Form 681 (Religious Exemption form Immunization, i.e. Religious Exemption), obtainable through the DOH Pinellas County/Public Health Unit.|
You should see if that is something they do there.
