That's not unusual. I think I posted about it a few years ago as well, after the Saba Button case and Fluvax fiasco. In subsequent years, GPs were still adminstering a product that was banned for under 5s to children in that age group.

It's up to parents to be knowledgable about what products are approved for their children and for what age group before the visit and to verify it before proceeding. Parents can ask to see the vial beforehand. Most won't though and just go along with what the doctor has selected.

I suppose doctors are equally sloppy in the US. Trusting medical staff comes with a high price.
