My daycare provider wants me to fill out a form with the vaccinations my son has. Well, he doesn't have any. She says she is required to have me fill it out... I know there's some kind of form I can get which says I have religious or philisophical beliefs that prevent me from vaccinating my son. Can anyone point me in the right direction? I'm in San Francisco, CA.

Exemption portion is on the reverse of the certificate: http://www.dhs.ca.gov/publications/forms/pdf/pm286b.pdf

You can also exempt your child from the TB skin test requirement - there's a separate signature line.
