Are you sure you need one? We only stated that we object on religious grounds and that was it. No questions asked in our public school system. That was a few years ago and has worked well.

We need one. We do private school and they require a statement explaining.

mom to 4 kids, ages 20 mos to 11 years

We do need one as well. The following are the questions we need to answer:

Explain in your own words why you are requesting this religious exemption.
• Describe the religious principles that guide your objection to immunization.
• Indicate whether you are opposed to all immunizations, and if not, the religious basis that prohibits particular
immunizations


And this is my proposed answer:

We are requesting that our child be exempted from mandatory immunization because the administration of immunizations conflicts with the practice of our religious beliefs. Our religious tenets prohibit us from allowing the injection of foreign ingredients into our child’s body, which God has created in perfection. Our belief is that any and all immunizations would violate our child’s body.

I'd ask the opinion of a NYer; I believe NYC is the toughest on religious exemptions.
